AvMed Health Plans announced Monday that personal information of some 208,000 current and former subscribers may have been compromised by the theft of two company laptops from its corporate offices in Gainesville on Dec. 11.
The information includes names, addresses, phone numbers, Social Security numbers and protected health information, according to an AvMed news release.
AvMed says that those former and current subscribers who may be affected will begin receiving letters by Feb. 8.
AvMed arranged for current and former members to receive 24 months of identity protection under the Debix Identity Protection Network free of charge. Those affected will have the option to apply for the identity protection.
